Who We Are

Empower Sports Chiropractic (“ESC,” “we,” “our,” “us”) provides sports-oriented chiropractic, physical therapy, and massage services at 407 E Maple St, Ste 107, Cumming, GA 30040.

This Privacy Policy explains how we collect, use, share, and protect information through our website, online forms, messaging, and in-clinic operations. It also summarizes how we protect health information under HIPAA and points you to our full HIPAA Notice of Privacy Practices.

We are a low-volume, longer-session, cash-based clinic. We don’t sell personal information or PHI (health info). We share data only to deliver care, run our practice, comply with law, or with your direction.

Scope: Website Vs. Medical Information

• Website/marketing information (“Personal Information”) includes details you provide via contact forms, scheduling requests, newsletter sign-ups, reviews/testimonials, and information collected via cookies/analytics.
  
  
• Medical information (“Protected Health Information” or PHI) includes any health-related data we create or receive in providing care. PHI is handled under HIPAA. (See “HIPAA summary” below and our full Notice.)

Information We Collect

You provide: name, contact details, message content, appointment preferences, and, if you choose, limited health context needed to book or triage care.

Automatically collected: device/browser data, IP address, pages viewed, timestamps, and similar analytics signals (see Cookies & Analytics).

How We Use Information

• Provide services: schedule visits, communicate about your care, send intake forms, and manage your account
• Care operations: quality improvement, safety, training, and technical support
• Communications: service announcements, appointment reminders, and, only with consent, news or promotions
• Legal/compliance: detect/prevent misuse; respond to lawful requests; maintain required records

We do not sell your information. We share with:



• Service providers (hosting, EHR/scheduling, forms, email/SMS) under contracts or Business Associate Agreements where required
• Other providers involved in your care (with your direction or as permitted by HIPAA)
• Authorities, when required by law

Text Messages (SMS) & Calls

By providing a mobile number, you may receive care-related texts or calls (e.g., appointment confirmations, reminders, schedule changes). Message/data rates may apply. Reply STOP to opt out of non-essential texts. Care-critical messages may still be sent as permitted by law. We do not use automated texts for marketing without your consent.

Cookies & Analytics

We (and our providers) may use cookies or similar tech for essential site functions, performance metrics, and to improve the experience. You can adjust browser settings to limit cookies; some features may not work without them. Where legally required, we honor opt-out choices for targeted advertising and respond to recognized opt-out preference signals.

State Privacy Rights

Depending on your state of residence, you may have rights to access, correct, delete, or opt out of certain data uses. To exercise a request, email drcroskey@empowersportschiropractic.com with “Privacy Request,” your state of residence, and your request.



We’ll verify your identity and respond as required. (Note: PHI processed under HIPAA is generally exempt from state consumer privacy laws; use the HIPAA process below for medical records.)

HIPAA Summary (Your Medical Information)

When we handle PHI, we do so under HIPAA. We may use/disclose PHI for Treatment, Payment, and Healthcare Operations and for other limited purposes permitted by law (e.g., public health, legal requests). You have right to access, obtain copies, request amendments, request restrictions/confidential communications, and receive an accounting of disclosures. 

Data Retention

We keep information only as long as needed for care, operations, and legal obligations. PHI and certain business records are retained as required by applicable laws and professional standards. Website/marketing data is kept for shorter operational periods unless you ask us to delete it (where applicable rights exist).

Security

We use reasonable administrative, technical, and physical safeguards to protect information. No method of transmission or storage is 100% secure. If we’re legally required to notify you of a security incident involving your data, we will.

Children’s Privacy

Our website is not directed to children under 13, and we do not knowingly collect Personal Information from children under 13 online. Parents/guardians may contact us to request deletion of such information.

Our site may link to other websites or services. Their privacy practices are governed by their own policies.

We may update this policy from time to time. We’ll post the revised date at the top and, when required, provide additional notice.
